Appointment Check List

  1. Medical and vision insurance cards
  2. Current glasses, sunglasses, contact lenses or a copy of your most recent glasses prescription (Rx)
  3. A list of all current eye drops and/or oral medications you are using, including over-the-counter medications
  4. Contact information for your physician(s) and a list of any health problems you may currently be experiencing or have had in the past
  5. We are available to make appointments during regular business hours Monday through Friday or anytime online. We will provide you with a courtesy reminder call to ensure you remember your examination appointment.
